Our office does participate with BlueCross Blue Shield of SC. Please be prepared to pay any co-pay, co-insurance or deductible amounts.

MEDICARE:

Our office does participate in the Medicare program. You will be billed for any co-insurance and deductible.

MEDICARE ADVANTAGE PLANS:

Point Of Service (POS) and Private Fee For Service (PFFS) Plans are accepted. We do not accept Health Maintance (HMO) or Health Savings Plans.

Our office does participate with these carriers. Please provide our office with a current copy of your insurance card and come prepared to pay your co-pay at each visit.

NON PARTICIPATING PLANS:

Payment in full is expected at the time of your visit. Our office will file your insurance claim for you. Please provide us with a current insurance card.

SECONDARY INSURANCE:

We will file your secondary insurance one time only per visit provided you have supplied our office with your current insurance card. Please save your copy of the explanation of benefits from your primary carrier in case refiling is necessary.

GENERAL INFORMATION:

Missed Appointments - A $25 charge will be made for appointments missed without 12-hour notice.
Returned Checks - A $30 charge will be assessed for returned checks.
Accidents - Our office does not accept assignment for automobile accidents or other personal injuries. Payment is expected at the time of service.
Worker's Compensation - We do not accept assignment from worker's compensation carriers. Payment is expected at the time of service.
Collections - Balances that are 120 days or older are subject to Collection Actions.
Medications Refills - Telephone requests for refill of medications will take no less than 48 hours and there will be a fee of $10.00 per call.
Completion of Forms - Minimum $20.00 charge for form completion. Payment due when forms are presented. Allow 5 business days for completion.
